The patients responsibilities

Arrive 15 minutes early for your appointment, as you will be expected to read and fill out and initial consultation form. |

Wear loose comfortable clothing. |

Bring any Imaging or tests such as x-ray, scans or blood tests. |
|
Bring a list or try to remember what medication you are on or have recently stopped taking. |
|
If you have to cancel an appointment, please give 24 hours notice or a cancellation fee will be charged. |
|
Payment is required at the time of the consultation, including work cover and TAC patients. The clinic accepts cash, cheque, eftpos and credit card (visa, Mastercard, bankcard). |
